There are two forms available for Member Essentials Basic Leagues to gather information from the public. You will find two forms you can use:

  1. Junior League Membership Application
  2. Junior League Membership Interest Form


Required Access:

You will need the Administrator or Form Administrator admin access level.


Where to go:

  • Go to Admin > Member Management 
  • Select Form Builder 2.0

Steps to be taken:


  1. Find the forms.

  2. Select the eye Icon to find the URL

  3. This URL which is only available to logged in users:

  4. Request your League's Hierarchy_ID by e-mailing MemberEssentials@ajli.org

  5. Customize link:

  6. To make a link available to the public, the form needs to be edited with your League Hierarchy ID .

  8. Do set Notificationd on each form so an email is sent when someone submits a form



  9. Test to confirm working this link is working after you post to your website.

  10. Review submissions by clicking Entries button
