  • Checkbox - Create checkbox selections for end users to choose from.

  • Currency - Creates a field in which you can enter a dollar amount.

  • Date - Creates a field for users to enter a calendar date.


  • Display Text - Used to explain to your users what the objective of the form/field is. Use the instructions field to fill up this area.

 

  • Dropdown - Creates a dropdown field where users can choose between selections that have been added by the form creator.

  • Dropdown Yes/No - A dropdown with only two selection choices, being Yes or No.

 

  • Email - A field for users to provide their Email address.


  • File Upload - A field for users to upload a file (Note: File Upload fields will only appear on public forms if the system parameter c_allow_public_uploads is set).


  • Integer - Allows users to input an integer.


  • Memo - Allows users to create a memo.


  • Phone - A field for users to provide their phone number

  • Radio Button - Allows users to choose from radio button options created by the form author (much like the checkbox field type)


  • Rich Text - Creates a field in which users can input text in Rich Text format.

  • SQL Field - Use this option to create a query for display purposes only. Requires a knowledge of SQL

  • Section - Used as a header to divide specific sections in a Form


  • Short Text - Adds a short text box (most commonly used)


  • Terms and Conditions - A field displaying the Terms and Conditions of the form (When applicable).


  • Terms and Conditions: Checkbox - Display the Terms and Conditions of the form, with an added checkbox for users to agree.
